The following forms are required to be filled out and brought to your first day of class.

Important Dates

  • DAY ONE

    Location: 1940 E. Maple Rd. Troy, MI 48083

    Time: 9am - 4pm

    *It is preferred you wear all black to class. Please bring a blank notepad, lead pencil and your printed documents.

  • DAY TWO

    Location: 1940 E. Maple Rd. Troy, MI 48083

    Time: 9am - 4pm

    You MUST bring a model for Day 2 of class. The model must have no previous permanent makeup or eyebrow tattooing done. Model also must be in good health with no medical contraindications. Advise your model that the appointment could take up to 2 hours. It is preferred you wear all black to class. Please bring a blank notepad and lead pencil.

You must complete and bring with you the following to the course:

1. Before the course begins, students must gain a bloodborne pathogen training 

certificate. This training has a $19.95 fee and can be done here.

A copy of your up to date bloodborne pathogen training certificate must be brought to the first day of Shape Your Beauty training.

2. A copy of your Oakland County Permit $50.00 fee

  (Must be submitted to ehclerks@oakgov.com)

 *Permit processing can take up to 30 days but typically ready within a week.

3. A copy of your phone I.D

4. A copy of either your Hepatitis B Vaccination records or filled out waiver 

    (Typically it is the waiver because obtaining medical records can be difficult)
